To ensure your signature is valid, please follow these instructions carefully:

1. Download and Print the Official Form

• Only use the official petition form provided on this page

• Each person must sign the form themselves

• A maximum of 10 signatures per form. A minimum of 1 signature per form.

2. Fill in Your Details Clearly

• Write all details in BLOCK LETTERS

• Complete all required fields

• Ensure your information is accurate and legible

3. Sign the Form

• You must provide your original handwritten signature

• Photocopies, scans, or digital signatures will NOT be accepted

4. Eligibility

• You must be over 18+ yrs old and enrolled to vote in New Zealand

• Only eligible voters can sign the petition

​

5. Return the Original Form

All completed forms must be returned as original hard copies.
